Chronic Disease Management Plans

Chronic diseases (such as type 1 diabetes) tend to require more visits to health care professionals, so Medicare offer rebates to help people better manage their condition. With a Chronic Disease Management Plan (CDMP), you can get $60.35 back from Medicare for up to 5 appointments per year.

 

To get a CDMP, you'll need to see your GP. From when it is created, a plan is is valid for 1 year . They usually require a longer appointment, so check with your GP practice when you're booking.

 

Together with your GP, you will choose which allied health professionals will be of most help to you, and your GP will add them to your plan. For people with type 1, the list often includes (but does not have to be limited to):

  • Diabetes educators

  • Dietitians

  • Podiatrists

When you have an appointment at the Family Centre (in person or via telehealth), or with any other allied healthcare professional that's on your CDMP, bring a copy with you (or email it to us) and you will be able to get the $60.35 rebate from Medicare.

Please note that your CDMP needs to be dated the same day or before your appointment with your allied healthcare professional, otherwise you can't claim the rebate.
