CARE NAVIGATION SERVICES
The Type 1 Diabetes Family Centre’s Care Navigation Service offers personalised support for people living with type 1 diabetes and their families. We work alongside you to help navigate the emotional, social, and practical challenges of life with T1D, and connect you with the supports that feel right for your needs.
We understand that living with T1D can sometimes feel overwhelming or isolating, and knowing where to start isn’t always easy. Our care navigation service offers a supportive space to talk through what’s happening for you, explore your needs, and make the next step feel clearer by connecting you with the support options available.
This statewide service includes a free* 45-minute appointment, available either face to face at the Type 1 Diabetes Family Centre or via telehealth, giving you space to talk things through with someone who understands.

Session by a qualified counsellor who brings lived experience as a parent of a child with T1D.
*Thanks to the generous support of WA Health, we are pleased to be able offer a free 45-minute session for:
People who are newly diagnosed (within 18 months); and
Individuals aged 17–30
