Omnipod 5: The Future of Diabetes Management in Australia

Many people with type 1 diabetes often face a tough choice. We have to choose between two significant features: tubeless design and an advanced algorithm. Personally, I prioritize the algorithm. I find comfort in knowing that my pump can help correct my blood sugar in case I under or over-bolus. Additionally, the convenience of an algorithm helps me feel more relaxed when my activity level changes.


For others, the frustration of tangled tubing can be overwhelming. From getting caught on door handles to having to disconnect while at the beach, these inconveniences can be significant. That’s where the Omnipod DASH comes in, providing a tubeless solution without an algorithm. However, that all changes come March 17, when Omnipod 5 releases both a tubeless option and an advanced algorithm!


The Headline Features of Omnipod 5


1. Automated Insulin Delivery with SmartAdjust™ Algorithm


The Omnipod 5 features the SmartAdjust™ algorithm, a notable upgrade from the previous Omnipod DASH. This innovative algorithm operates directly on the pod and communicates continuously with a compatible Continuous Glucose Monitor (CGM). It predicts glucose levels up to 60 minutes ahead, allowing it to automatically adjust insulin delivery. This means it can increase, decrease, or pause insulin to help keep blood sugar within your target range. In theory, this should reduce the number of highs and lows to worry about, leading to better overall management—and a more restful night’s sleep!


When you set up the pump, you specify your target blood glucose (Target BG). It's crucial to know that this setting is the only one influencing how the algorithm manages your doses. You can select target levels of 6.1, 6.7, 7.2, 7.8, or 8.3, with up to 8 different targets programmed for various times of the day.


During the setup, you’ll also configure values for Correct Above, Insulin to Carb ratio, Correction Factor, Duration of Insulin action, and Reverse Correction. These configurations help with the Smart Bolus calculator but do not influence the algorithm’s automatic adjustments—only the Target BG does.


Basal rates are programmable too, but these are only relevant in manual mode or if the pod loses CGM connection for an extended duration. When automation activates (typically upon starting your second pod), it ignores your original basal rates and switches to the Adaptive Basal Rate calculated specifically for you. This learning process adapts with each pod change, leading to promising results. Anecdotal reports suggest a somewhat rocky first week as the system learns, but things smooth out after 1–2 weeks.


2. Cloud Connectivity


With Omnipod 5, cloud connectivity is a new feature that many users will appreciate. Unlike the Omnipod DASH, which doesn’t sync data to an online platform, the Omnipod 5 will automatically upload your data to the cloud. This action simplifies data sharing with your healthcare team and offers valuable insights into your diabetes management. No longer will you have to hunt for that specific cable the night before your appointment to upload your data!


3. The Controller


You can still adjust the Omnipod 5’s settings using a proprietary controller (note: there’s currently no phone app available!). However, the good news is the controller does not need to be nearby all the time. The algorithm functions independently on the pod itself, which allows freedom of movement. You can even enjoy activities like swimming without needing to constantly carry the controller. The pod consistently updates with CGM information and stays in “auto mode,” working to maintain your range, even when the controller's out of reach.


In the US, a smartphone app is already available for users, making it even more convenient. Unfortunately, we won’t have access to this app at the launch here in Australia, and predicting when it may arrive is uncertain. If you’re considering switching to Omnipod 5 solely based on this app, it might be wise to expect at least a two-year wait.


CGM Compatibility


We’ve long advocated for more choice in compatible devices, and now Insulet is delivering! With Omnipod 5, compatibility expands to include Dexcom G6, G7, and the soon-to-be-released Freestyle Libre 2 Plus. As of now, we still lack firm release dates for the G7 and Libre 2 Plus, but rumors indicate they will become available "soon!"


What’s Happening to Omnipod DASH?


Not to worry—Insulet has confirmed that DASH is not being phased out. If you’re not ready to fully embrace the automation of the Omnipod 5, you may continue using Omnipod DASH for the foreseeable future. Can You Trial Omnipod 5 Before Committing for Four Years?


The beauty of the pod-based system is its flexibility. Since every pod acts as a new pump, you can purchase a small supply (say, 1 or 2 months’ worth) to test its suitability before committing financially to the longer-term contract.


Previously, Insulet has offered promotions—for instance, a $90 fee for a 90-day supply of pods. While we hope to see similar offers return, we currently lack any official announcements.


Pricing and How to Get Started with Omnipod 5


Even if you're currently using Omnipod DASH, you will still need to see your diabetes educator to transition. Each individual’s transition will differ. Below are the detailed steps and pricing information for various scenarios.

Current Omnipod DASH User Accessing via Private Health


You don’t need to wait until your private health warranty is up to transition from DASH to Omnipod 5. Upgrading will not reset your warranty.


To access Omnipod 5, you’ll need:

  • A letter of clinical need from your Credentialled Diabetes Educator

  • A completed order form, signed by you and your Credentialled Diabetes Educator

  • A one-time payment of $249 to Insulet for the upgrade from the PDM to the Controller

  • A pump start training session with your Credentialled Diabetes Educator


You can continue to obtain pods through NDSS for the current monthly cost of $31.80.


Current Omnipod DASH User and Self-Funding


For private funding access to Omnipod 5, you’ll need:

  • A letter of clinical need from your Credentialled Diabetes Educator

  • A completed order form, signed by you and your Credentialled Diabetes Educator

  • A new contract with Insulet including a $185/month access fee

  • A pump start training session with your Credentialled Diabetes Educator


You will continue to access pods through NDSS for $31.80 monthly.


Completely New to Omnipod and Accessing via Private Health


Your educator will require a specific level of pre-pump education. This might involve a few clinic sessions before starting your pump.


To access Omnipod 5, you’ll need:

  • A letter of clinical need from your Endocrinologist—this is a requirement by Private Health Companies

  • A completed order form, signed by you and your Credentialled Diabetes Educator

  • A pump start training session with your Credentialled Diabetes Educator


Then, you can access pods via NDSS for $31.80 monthly.


Completely New to Omnipod and Self-Funding


As with the private health option, you’ll need a certain level of pre-pump education, which may involve several clinic sessions.


To access Omnipod 5, you’ll need:

  • A letter of clinical need from your GP

  • A completed order form, signed by you and your Credentialled Diabetes Educator

  • A contract with Insulet including a $185/month access fee

  • A pump start training session with your Credentialled Diabetes Educator


You can then access pods through NDSS for the current monthly price of $31.80.
