Patients can self-refer to the Type 1 Diabetes Family Centre simply by contacting us. Healthcare providers can also refer patients directly to our allied health services and patient education programs.

Every person with type 1 diabetes is eligible for a Chronic Disease Management Plan (CDMP) from their GP. A CDMP will subsidise visits for your patient when accessing our diabetes education and dietetics services.
If you are creating a CDMP for a patient you can:

Simply fax or email the CDMP to us using the contact details at the top of the page
